MS5 HRSN provides essential support services to Oregon Health Plan (OHP) members facing housing instability, climate-related hardships, and access barriers. We offer housing assistance, outreach and engagement, case management, and climate support to help individuals stay safe, connected, and stable.
MS5 Outpatient Clinic – Coming Soon!
MS5 Outpatient Clinic will be a licensed behavioral health center offering culturally competent mental health services. We will provide therapy, peer support, case management, and trauma-informed care to individuals and families — with a focus on those historically underserved by traditional healthcare systems. Our goal is to close critical gaps in care and build long-term wellness in the community.
