
Who We Are
MS5 is a community-based organization committed to helping Oregon Health Plan (OHP) members access the critical support they need to live safe, stable, and healthy lives. We specialize in delivering Health-Related Social Needs (HRSN) services to underserved communities across Oregon — including people experiencing homelessness, those at risk of housing loss, and individuals impacted by poverty, climate hardship, or language barriers.
We are proud to serve everyone with dignity and compassion, with a deep focus on supporting Arab, Muslim, LGBTQ+, immigrant, and all other marginalized populations.
What We Do
We provide a wide range of services, including:
Housing and Utility Support
Outreach and Engagement
Case Management
Climate Support (AC units, heaters, purifiers, etc.)
We work directly with clients, landlords, case managers, and CCOs to remove barriers and meet urgent needs — quickly and effectively.
Our Vision
Our long-term goal is to expand our services statewide and establish a licensed outpatient behavioral health clinic to further support our community with therapy, peer support, and culturally informed mental health care.

MS5 was founded on lived experience, community care, and the belief that everyone deserves access to safe housing, basic needs, and trusted support — no matter their background or circumstance.
